Pulmonary Rehabilitation Questionnaire

  1. Have you been diagnosed with COPD, chronic bronchitis, cystic fibrosis, asthma or emphysema?

  2. Does shortness of breath limit your ability to complete tasks or participate in activity?

  3. Do you have a chronic couch which may or may not be productive?

  4. Have you been prescribed oxygen therapy?

  5. Do you have a history of smoking?

  6. Do you have frequent respiratory infections?
