2. If you do not have Activation Code, click “SIGN UP ONLINE” on right side of the page

3. Fill out form fully and click “Submit”

4. Answer security questions

5. Set up username and password (Email addresses can only be associated with one MyChart account. Save your username and password in a secured area or password protected note.)

6. Download app in Apple App Store or Google Play Store to enjoy all the features, below, that MyChart has to offer

Things to Keep in Mind Before You Call or Contact

1. Patients who do not have a social security number in our Electronic Medical Record system will be unable to create a MyChart account. Please have that number available when you contact.

2. Patients with ages 12-17 will be unable to create/access MyChart (This is an automatic equation built into MyChart and is something Nor-Lea Hospital District is unable to control.)

3. If you have a shared email address with a spouse or family member, the email address should only be used in association with one MyChart account.
