Helpful Tips
Kids are welcome! However, if you have more than 2 children with you, we encourage you to bring an extra adult to help throughout the appointment process. Keep in mind, children under 14 cannot be left unoccupied anywhere on our campuses.
Please refrain from using your cellphone during your appointment. Our providers are here to help in full capacity and welcome attentive conversation.
Manage your appointment through MyChart! Refill medication, communicate with your provider’s office, view upcoming appointments, request medical record copies, pay your copay and view test results with the MyChart app. Click here for more information about MyChart, including downloading instructions.
What are my Clinic’s Hours?
Scheduling & Cancellations
Please arrive 15 minutes early for your appointment, being on time prevents delays within your office visit and allows enough time for a through registration process.
Please contact us to cancel or reschedule.
If you are unable to attend your appointment, let us know! This allows us to assist other patients in your absence.
What do I need to bring to my visit?
Bring your insurance information to every visit.
This ensures we are billing correctly and that we have the most up to date information to prevent delays in service.
Bring your medication and a list of any medication allergies.
This is important to prevent medication errors and ensures accurate and up to date medication to your chart for continuity of care.
We request that you bring a current form of identification.
State drivers license, etc.

ZERO TOLERANCE BEHAVIOR
Nor-Lea is a weapon, smoking, vaping and alcohol-free campus. This is for patient and staff safety as well as the health of our patients and staff. Swearing, threats or any act of violence will NOT be tolerated. Anyone giving verbal abuse to members of staff will be asked to leave immediately. No weapons, smoking, vaping or alcohol.
