Qualifications
Patients must be medically stable
Patients must have three consecutive inpatient midnights within 30 days
Prior Authorization from commercial insurance is required prior to acceptance out of state Medicaid is not accepted
Must have two qualifying factor needs such as:
Physical Therapy/Occupational Therapy
Physical Therapy/Speech Therapy
Physical Therapy/Wound Care
IV Therapy/Wound Care
What is the Swing Bed Program?
Nor-Lea Swing Bed Program offers temporary skilled care until you are able to return home safely.
Medicare, Medicaid and some private insurance programs will pay for qualified skilling care.
Skilled Care is when a patient requires daily treatment or supervised care from a licensed professional nurse, therapist or physician and shows progress towards a goal after a three night acute stay.
