Services We Provide

Chronic Care Management for Medicare patients with two or more chronic illnesses recognized by Medicare as potential cause for declining health or increased ER visits or hospitalization.

Transitional Care to help patients transition form the hospital to the home, including assistance with making follow-up appointments, ensuring ordered medications and/or equipment are received and providing education on symptom management and home recovery.

Annual Wellness Visits are preventative screening which are paid for by Medicare without co-pays or out of pocket expenses. These are conducted once year and are coordinated by an Annual wellness Visit expert and your physician. Preventative screenings may include screenings for the following disease process.

  • Breast Cancer

  • Prostate Cancer

  • Colon Cancer

  • Osteoporosis

  • Diabetes

  • Depression and Safety Risks
